5d50368b38 Update KA10 emulator. 
The emulator has an update with regards to POP P,P.  The new behaviour
will store the top of the stack into the P accumulator.  This is
required to run old versions of Muddle, up to around version 54.

e6534c1362 Update tv11 emulator and throttle it to use less host CPU. 
The -s argument is number of milliseconds to sleep ever 1000 PDP-11
instructions.

8cef6e56d9 Make a KLDCP front end directory. 
Having the .KLFE.; KLDCP (DIR) file first on the tape practically
guarantees SALV will make store it in contiguous sectors on disk,
which is required by the front end.

0868cf6eaa For KA10, make MAGDMP tape bootable. 
There is no longer any need for a MAGDMP paper tape.

b2f52fc28c Use relative paths for tools in the Makefile. 
This avoids problems when the full path to the build directory has
spaces in it; make needs spaces in target/dependency names to be quoted.
If the paths are all relative, then we only need to use $(CURDIR) (and
quote carefully) when adjusting PATH, etc.
