4a7182d9ed Fix SIMH configuration for GT40. 
Set device unit DLO0 to 8-bit mode to ensure keyboard codes are
transmitted.

Set switch register to 0 to get a blinking cursor and roman text.

579a3030d7 Add VT11 interrupt vector for GT40 to work. 
It should be 320 according to VT07.
